About Caregiver Plan
Caregiver Plan provides an easy to follow, week by week three part program that helps caregivers plan the care they will provide their loved one. The three parts are:
- Plan: A twelve step care plan with important issues highlighted each month, as well as vital information about support systems, financial and medical information, and safety to build a comprehensive plan of care.
- Activity programming: monthly activities, content and resources in the fields of nutrition, fine arts, family history, and movement.
- Support community: a virtual community; a place where family and professional caregivers can come together and share about their experiences (the good, the bad, and the ugly). This includes a private facebook group, our signature “cuss and discuss” weekly discussions, and disease management programming and resources
Caregiver Plan was born out of necessity. When our founder and home health professional, Samantha Young, saw that there wasn’t one place to find information or training for professional and family caregivers, she decided to create an online solution.
Caregiving is one of the most enriching experiences you’ll ever have but without the proper preparation and guidance it can also be one of the most challenging. Our subscription helps you through the process by providing invaluable resources and activities from home health experts. Our mission is to help you take care of yourself while providing the best care for your loved ones.
Our logo is based on the monarch butterfly because, like a monarch butterfly, the process of becoming a caregiver has phases. You start as a beginner, go through the developmental stages, and eventually become an expert. Learning how to be a caregiver is a process and we are here to help you every step of the way.
Our team brings thousands of hours of experience working with senior populations. Every Caregiver Plan content creator is an expert in their field.
Adrien Pacsoza: Nutrition
Adrien is a Registered and Licensed Dietitian and a Certified Eating Disorders Registered Dietitian Supervisors (CEDRD-S) practicing in Austin, Texas. She is active in local, state and national dietetic associations through volunteering and holding board positions, such as Texas Academy of Nutrition and Dietetics Media Representative, Chair of Behavioral Health Nutrition (BHN) Dietetic Practice group, and President of Austin Dietetic Association.
Alyssa O’Krent: Movement
Dr. O’Krent is passionate about her work, traveling, and music. Her goal is to help patients unlock their potential by educating them along the way with simple, key concepts on how to progress their exercise routines and activity levels. Dr. O’Krent strives to provide the best orthopedic care to her patients by improving their strength, general conditioning and overall wellness.
Damien Temperley: Brain Games & Movement
Damien has been in the fitness industry for over 18 years with the past 8 years focusing on older adults. Damien creates and teaches a variety of classes including strength building, balance improvement, core workouts, stretching, and memory and brain games geared to adults 60+.
Jackie Hall + Stacy Colston: Art
Stacy and Jackie enjoy creating their own artwork and teaching Art to the young at heart! They are both certified art teachers in Austin, Texas and together have over 30 years of teaching experience.
Mike O’Krent: Family History
Mike O’Krent is the CMO (Chief Marketing Officer) at caregiver Plan and the founder of LifeStories Alive, LLC, a business specializing in producing living stories for families and businesses.
Samantha Young: Eldercare
Samantha Young, is a Licensed Nursing Facility Administrator and is the Founder and CEO of CareGiver Plan. An idea that began ten years ago after noticing through her own caregiving experience that there was no one place to find information and training for caregivers.
